New Client Booking Form

We are delighted that you would like to enquire about our psychology practice. So that we can ensure that we match you with the best clinician, please provide us with as much information as you feel comfortable with over this secure form.

Questions marked with an asterisk are required and if you have concerns about providing information in this format, we can talk about this over the phone.

Once you have completed this New Client Form we recommend you phone the office to complete the booking so we can match you to clinician experience and availability.



New Booking Service Terms

  • I understand that the practice' requires direct debit payments on the day of service, that cash is not accepted and there is a late notice cancellation fee of $150* for sessions cancelled or rescheduled within 48 hours.

  • I understand that I must book at least 4 sessions ahead on a weekly basis initially. Aresolution has a duty of care to be able to provide an adequate service which means follow-up appointments are paced in a meaningful way. I know that the psychologist understands if I do not wish to continue after the initial consultation and will facilitate transfer to another more suited psychologist or cancel future booked appointments. 

  • I will make payment of the $30 new client booking feee (only 1 fee is charged for the new booking even if you book your 4 or more appointments). I understand that this fee is non-transferable or refundable and my appointment will be canceled if the booking fee is not received promptly.

  • I understand that if the appointment I am booking is for a child under 12, the parent/guardian must attend the initial session (booked in the child's name) and that if I have exceptional circumstances and would like to change the parent session to the second session I will contact the office to make the booking for my child. I will complete the parent intake questionnaires. 

  • I understand that if I intend on claiming Medicare rebates (Mental Health Care Plan, Eating Disorder Plan, GP Management Plan, etc) I will email my GP referral letter to admin@aresolution.com.au within 24 hours or phone reception to advise the date I intend to receive it. I understand that I am responsible for tracking sessions on my referral and Medicare rebate processing is a service provided by Aresolution and they cannot be held responsible for errors.

  • I understand that appointments for psychological assessments must not be made online and need to be made through the Aresolution office so that the psychologist can be matched to the reason for my report, my diagnosis, treatment history and deadline for the report.

  • I understand that if I have been admitted to a hospital in the last 3 months for a mental health concern, if I wish for invoices to be sent to my insurer, Work Cover or my NDIS plan manager, if my appointment relates to eating disorders, borderline personality disorder, bi-polar disorder or schizophrenia I need to phone the office instead of making an online appointment so that the practice can make the appropriate booking with the right clinician and be informed of the correct paperwork required for the booking. Clinicians' capacity to assist clients varies according to their caseload.

  • I understand that the practice prefers phone bookings and that if I make an online booking it will be reviewed in line with the clinician's capacity for new clients and the provision of ongoing sessions to myself and their existing clients and that in some instances I may be asked to reschedule my initial appointment to a later date. 

  • I understand that the practice prefers 75 minute initial appointments but offers 50 minute initial consultations if there is limited availability. 



General Information

New Child Clients

If your appointment is for your child (13 and under):

  • Make the booking in your child’s name (include your name in the booking notes)

  • We require parents to attend the first session on their own

  • If you have Court orders email them to reception within 24hrs of your booking

  • Call reception to alert them of the booking if you intend for the parental consultation to occur after the first appointment

  • Complete the parent intake forms and parent consent forms.

  • If your child is under 13, please do not bring them to the initial consultation as we do not recommend they sit unattended in the waiting area

  • Please email any documentation 1 week prior to your session
